The template is an Excel file. We recommend saving a new tab for each month, updating your priorities and outcomes as you go, and updating or clearing the previous month's content to keep it clean and focused.

By the end of the year you will have a complete record of twelve individual months of goals, priorities, actions, and outcomes ready for any performance or development conversation.
